(C) Directorships - if you are a remunerated director of a company or companies, give the name(s) of those companies
(A) Election expenses - if you have received any payment in respect of your election, give the name of the person or the body making the payment (unless it is the GLA, a London borough or other relevant authority) - but you need not include the amounts.
(B) Other expenses - if you have received any payment in respect of any expenses incurred by you in conducting the business of LFEPA, conducting any business as a Member of LFEPA or acting as a representative of LFEPA, give the name of the person or the body making the payment (unless it is LFEPA, the GLA, a London borough or other relevant authority)- but you need not include the amounts.

(a) you own investments in any person or body and your interest exceeds the nominal value of £25,000 or 1 per cent of the total issued shared capital; and

(B) a firm in which you are a partner (ie, a firm listed under 2A above)

List all persons from whom you have received a gift or hospitality with an estimated value of £25 or more in the last three years [note: applicable from 21 September 2007 when the revised code of conduct came into effect].
List any body of which you are a member or in a position of general control or management and to which you are appointed or nominated by LFEPA.
List any body of which you are a member or in a position of general control or management that:
(A) exercises functions of a public nature; [note: this includes membership of the Greater London Assembly or of a London Borough Council]
(B) is directed to charitable purposes;
(C) of whose principal purposes include the influence of public opinion or policy [note this includes political parties and trade unions].
[Notes: (i) you do not need to include here any shareholdings (see 4 above) nor any building society in which you have investments or from which you have borrowed money. (ii) The Standards Board for England advises that Members who are Freemasons must register their membership of the Grand Charity].
